F10 Notification of a Construction Project

Accessibility Statement

This form's layout takes into account visually impaired and 'hands-free' user needs. We have designed it to be as accessible as possible. It is compatible with popular screen reading and voice recognition software and you can navigate it using the keyboard only. If you are using older versions of the adaptive equipment software, you may find, although the form is accessible, it may not perform as well as with later versions.

Form overview

The form is four pages long and can be navigated using the Next and Back buttons on each page. You can submit an initial notification or update a previously submitted notification. Help is provided on each screen and can be accessed by selecting the ? buttons.

Voice recognition users may select the help buttons by:
  • using the commands "Form Level Help", "Screen Help" and "Keyboard Shortcuts".
  • as a general rule, using the field name followed by "Field Help" e.g. "Planned number of contractors field help" for field level help.
  • using the command "Address Help" for guidance on addresses.

  • Screen 001:
    Enter details of the site address and local authority.

    Screen 002:
    Give details of the construction project. Enter data for the project description, description of the construction work, start date, duration of the construction phase, and number of people on site.

    Screen 003:
    Enter contact details for Principal Designer, Principal Contractor, Client, Designers and Contractors engaged on the project.

    Screen 004:
    Select the relevant declaration and enter your name and role. Press the Print/Save Form details to print or save a copy of the data you have input to the form. Press the submit button to submit the form to HSE.

    Who should complete this form?

    This form should be completed by the client, or someone acting on behalf of the client, to notify the enforcing authority for the Construction (Design and Management) Regulations 2015 of any project that is likely to last longer than 30 days and have more than 20 workers working simultaneously at any point on the project OR is scheduled to exceed 500 person days on construction work.

    Any day on which construction work is carried out (including holidays and weekends) should be counted, even if the work on that day is of a short duration. A person day is one individual, including supervisors and specialists, carrying out construction work for one normal working shift.
