The law requires responsible persons (as defined) to report certain incidents to the relevant enforcing authority. For most incidents other than those on railways, reports can be submitted using the appropriate HSE online form.
You should not use this form if you are not the Responsible Person, or for non-reportable incidents. Other serious concerns about workplace safety may be reported to HSE in accordance with the complaints procedure http://www.hse.gov.uk/contact/concerns.htm.
The completed form is electronically sent to the appropriate enforcing authority on Submit.
